As I assume most of the above are detailed in Starfire's ISW4 and Crusade
releases, it begs the more general question, 'has anyone done a general
Starfire-to-FTII conversion system?'
I seem to recall some discussion, but nothing definative.

Not just "most" of them; *all* of them. Plus a bunch of ships that only get at
best a mention in passing in the books, and in some cases not even that. Those
books were based on the scenario modules, not the other way around...

A couple of ships are missing. The Ophichui (sp?) CVE mentioned in the
racial section of ISW-4 and in some flavour text, but never actually
used in combat.
The same with various intermediate refit designs for all races.
As explained when I asked about this on the Starfire mailing list, they
deleted any design that never appeared in a scenario.

The question originally was for the books; I don't recall tweety CVE's, but I
could well be mistaken.
Or, as Oerjan should have said, 'not to put too fine a point on it...'
The Design Bureau still has ISW4 and Insurrection info, but in electronic
only. Crusade can't be given away.
